Neck pain

A stiff neck you can't turn, an ache that runs into the top of your shoulder, a headache that starts at the base of your skull — neck pain is common, usually not dangerous, and often tied to hours at a screen, a bad night's sleep, or a jolt in traffic. Here's what's commonly going on, what helps this week, the straight answer on whether neck adjustments are safe, and when it's time to be seen.

You're in the right place if…

  • You can't check your blind spot without turning your whole body.
  • It's stiff every morning and loosens by lunch, then tightens again by evening.
  • Your headaches start at the back of your head or behind one eye after a long day.
  • Pain or tingling runs into your shoulder blade or down an arm.
  • You were rear-ended, felt fine, and then woke up stiff a day or two later.

What's commonly going on

The neck holds up a twelve-pound head all day, and the further forward the head drifts, the harder the small joints and muscles at the back of the neck have to work. Most neck pain comes from those joints and muscles getting irritated, sometimes with the mid-back stiff underneath and doing too little. Pain into the arm, especially with tingling or numbness, suggests a nerve is being irritated where it leaves the spine; that changes the evaluation and sometimes the plan. After a car accident, symptoms often show up 24–72 hours later; the sooner it’s evaluated, the easier it is to guide.

What you can try this week

  • Bring the screen to eye level and sit back in the chair; a laptop on a stack of books and a separate keyboard beats a new chair.
  • Every 30 minutes, turn your head slowly to each side and look up and down, within comfort.
  • Heat on the neck and upper shoulders for 10–15 minutes in the evening.
  • Sleep on your back or side with a pillow that keeps your neck level, not propped up; stomach sleeping is the usual culprit.
  • Gentle chin tucks and shoulder-blade squeezes, five slow reps, a few times a day.

When to be seen — and when to go straight to a physician

Go to a physician or emergency care today if neck pain follows a significant injury, comes with fever or a stiff neck you can't flex, severe headache, dizziness, trouble speaking or swallowing, weakness or numbness in both arms or in the legs, or chest pain.

Book an evaluation if it’s limiting your movement, lasting more than two weeks, running into your arm, or recurring.

Is neck manipulation safe?

This is the question people are most afraid to ask, so here is the answer. For most people, side effects of neck manipulation are mild and short — soreness or stiffness for a day or two. Serious complications are rare. Dr. Brandon will ask about your history — high blood pressure, stroke, blood thinners, connective-tissue conditions, prior spinal surgery — before recommending it, and neck manipulation is never done without discussing it with you first. If you’d rather not, say so: low-force techniques, mobilization, soft-tissue work, and exercise are all options, and many patients do well without a traditional neck adjustment.

What an evaluation at FIT Clinic looks like

History first: how it started, what it’s stopping you from doing, any arm symptoms, any accident. Then movement — how far you turn, tilt, and look up and down — plus checks of the mid-back and shoulders, strength and sensation in the arms if there’s any nerve involvement, and palpation of the joints and muscles. You’ll leave with a plain-English explanation and options.

What care may include, and what the evidence says

Care typically combines mobilization or adjustment of the neck and mid-back, soft-tissue work, and specific movements, with practical changes to your desk and sleep. Research suggests spinal manipulation can help both acute and chronic neck pain, generally in combination with exercise; results vary by person and the evidence isn’t uniformly high quality. If arm symptoms don’t improve as expected, we refer for imaging or a physician’s evaluation.

When stress is part of the picture

Stress keeps a lot of people braced: shoulders up, jaw set, breathing high in the chest. If that sounds familiar, biofeedback training teaches your body to downshift, with live data. It works alongside an evaluation, not instead of one.

Questions

Will my neck be "cracked"?

Only if you're comfortable with it and it's appropriate. Ask for low-force options at any point.

I was in a car accident — should I see you or my doctor first?

If you have any of the warning signs above, a physician or ER first. Otherwise, an evaluation within the first week is reasonable; we'll refer if anything needs imaging.

Can neck problems cause my headaches?

They can; that's what the evaluation sorts out. See our headaches page.
